• "Q"- PON must be presented at time of visit/fitting
• Standard contact lens fitting with spherical contact lenses includes first pair of trial lenses
• Offer not valid with toric (astigmatism-correcting) or multi-focal lenses, but may be used for monovision (one eye corrected for distance and one eye corrected for near vision) contact lens fittings
• Does not include eye health examination which is required prior to contact lens fitting; Deal may be used with vision and/or medical plans for eye examination only. Patient is responsible for co-pay or deductible associated with insurance plans.
